Pastoral Counseling Request Form

Thank you for your interest in setting up a counseling appointment.  Please complete the form below.  There is no fee for active members of WOCC.  However, non-members and inactive members will be accessed a fee of $35 per session.

Once you have submitted this form, you will be contacted by one of our Ministerial Staff or Church Officers to set up your appointment and collect any required fees.  Upon confirmation of your session, you will be given a Zoom Code for your session.  

If you need to reschedule or cancel your confirmed session, you must do so with at least 48 hours advance notice.  Otherwise, please log into your session 5 minutes prior to your scheduled time.  A 5 minute "Grace Period" is give for all sessions.  After that, your session will be considered a "No-show" and will be automatically cancelled.  No-shows and cancelations made less than 48 hours in advance will be assessed a fee of $25 which must be paid before a new session can be scheduled.
